If you are asking about a total custom theme for your site yes those do cost more but they are worth every penny the cost depends on how much work you have Russ and Steve do. Most of the themes for sale by Pixel Exit are pretty easy to customize on your own. Here are a couple of threads that show what people have done with Pixel Exit styles as well as styles custom made by Russ and Steve for various sites I hope this helps.
